It is great that value-based care is gathering momentum. In my view it is necessary to reduce, maybe even reverse the inflation in US health-care costs.

While we are seeing increased interest on the payer side, the large hospital systems aside, many small- and medium-sized practices remain ill-prepared to thrive in a VBP world. Among providers in the Central Bronx, data analytics, financial/cost accounting tracking, and physician alignment remain major barriers to progress.

John, that is a very fair comment about the difficulties that small and medium-sized providers face. Are you seeing the providers in your area trying to form affiliations or other arrangements to create more economies of scale to better be able to afford the tools needed for VBP?

Choice of model is critical. Many, if not most (if not all), models of value-based payment result in a focus on a narrow band of measurements and targets and incentives, to the detriment of a broader focus on health of a patient or population. Some would say that there is no pathway to perfection through value-based payment, but I'm still looking for that perfect model ....
