“Kent State University hereby requests proposals from appropriate medical laboratory service providers to perform all of the non-CLIA-waived laboratory services listed on the attached specification and proposal form. The laboratory services offered through DeWeese Health Center will be predominantly a cash pay model and focus is on client bill pricing. If patients do wish to use insurance, we will provide them a written lab order for a reference lab of their choice. Service
provider will also provide an on-site phlebotomist to collect specimens and coordinate services for the DeWeese Health Center during hours of operation and also meet all professional standards and Federal and legal requirements in the fulfillment of such services.

Proposers are asked to provide client bill pricing on all of the tests listed on the attached specifications and proposal form according to CPT code. The frequency of testing for each code is also indicated for fiscal year 22. In addition to providing pricing, as well as fulfilling all of the other obligations enumerated elsewhere in this document, respondents are asked to propose other services—along with costs, if appropriate—which are deemed advantageous to DeWeese Health Center’s commitment to provide quality healthcare and healthcare education for the campus community.”
